These surveys
collect information about the delivery of services from FSA to its
customers. Personally Identifiable Information is not collected.
E-mail addresses are input into the system in order to send e-mails
to invite customers to the URL to answer the surveys.

This information
is being collected in order to inform FSA how it can improve its
services to their customers.
