These surveys collect information about the delivery of services from FSA to its customers. Personally Identifiable Information is not collected. E-mail addresses are input into the system in order to send e-mails to invite customers to the URL to answer the surveys.

This information is being collected in order to inform FSA how it can improve its services to their customers.

FSA uses this information to improve its services to its customers.

No

All surveys are voluntary. A statement is included at the top of the invitation e-mails that describes what information is collected and how it is used.

The e-mail information input into the system is protected by firewalls and restricted access to the information.

No
